Community Mental Health Team (CMHT)

All referrals from the Adur CMHT need to have a Fair Access to Care Assessment (FACS) to assess their eligbility for social care funding.  If needs are asessed as substantial or critical the service will be funded through Self Directed Support (SDS).

If needs are FACS assessed as moderate, you will still be eligible for a service, under different funding arrangements.
